An algorithm for treatment of the drooping nose.

چکیده
UNLABELLED Nasal tip ptosis ("drooping" or long nose) occurs when the tip of the nose is more caudal than what is deemed ideal. Intrinsic factors, such as elongated or caudally-rotated lower lateral cartilages, can lead to nasal tip ptosis. Extrinsic factors, such as elongated upper lateral cartilages or excessive caudal anterior septum and heavy nasal skin, can push the nasal tip caudally and lead to drooping of the nasal tip. The loss of maxillary or nasal spine support may enhance the potential for tip ptosis. In addition, a hyperactive depressor nasi septi could, as a result of continuous pull on the tip, result in tip ptosis. Aging or previous nasal procedures (such as the Goldman-type tip surgery) where the continuity of the lateral and medial crura of the lower lateral cartilages have been violated may cause a weakening of the tip-supporting mechanisms and de-rotation of the nasal tip. Correction of this deformity is challenging and rewarding; it can resolve both the cosmetic deformity and nasal obstruction symptoms related to this entity. The goal of this article is to present our current principles of diagnosis and treatment of nasal tip ptosis, as well as to introduce and algorithm of preferred methods and techniques for its reliable and stable correction. RESULTS Correction of the nasal tip ptosis requires accurate diagnosis, a recognition of the interplay between various anatomic components, specific strategy planning, and a correction of anatomic abnormalities.

Approach to the correction of drooping tip: common problems and solutions
The drooping tip deformity is both a bothersome aesthetic feature and functional impairment of the nose. Both static and dynamic factors may affect tip appearance and it seems logical to take into account these factors when planning correction of drooping tip. Many studies have examined this topic, but its treatment remains controversial. In order to make nasal tip surgery successful, it is use...
متن کاملMultifactorial Approaches for Correction of the Drooping Tip of a Long Nose in East Asians
A long nose with a drooping tip is a major aesthetic problem. It creates a negative and aged appearance and looks worse when smiling. In order to rectify this problem, the underlying anatomical causes should be understood and corrected simultaneously to optimize surgical outcomes. The causes of a drooping tip of a long nose are generally classified into two mechanisms. Static causes usually res...
متن کاملSkin Excision as an Adjunctive Technique to Rhinoplasty in Middle-Aged and Elderly Patients
Rhinoplasty in middle-aged and elderly patients comes with its own set of challenges. There is relative lengthening of the nose with drooping of the nasal tip. With aging, the skin loses its elasticity, and the combination of nasal skeletal reduction along with overlying inelastic skin provides a setup for skin redundancy and poor postoperative outcome. We describe a surgical technique involvin...
متن کاملAutonomous Underwater Vehicle Hull Geometry Optimization Using a Multi-objective Algorithm Approach
Abstarct In this paper, a new approach to optimize an Autonomous Underwater Vehicle (AUV) hull geometry is presented. Using this methode, the nose and tail of an underwater vehicle are designed, such that their length constraints due to the arrangement of different components in the AUV body are properly addressed. In the current study, an optimal design for the body profile of a torpedo-shaped...
متن کاملRhabdomyosarcoma: Nose Presenting As Second Primary with Carcinoma Tongue
Introduction Presence of two primary malignancies is rare and occurs in 3-5% of the cancer patients. As per our extensive internet research, this is the only reported case of a synchronous sino-nasal embryonal rhabdomyosarcoma with squamous cell carcinoma-tongue. The case report is important because of the rare diagnosis and the challenge we faced in the diagnosis and treatment of the pat...
